You Don't Have a People Problem
Sometimes the team isn't the issue. The system is.

When the same mistakes keep happening with different people, stop looking at the people.
Look at the process they're being handed. Most "people problems" I'm called in for turn out to be missing steps, unclear ownership, or work that lives in one person's head.
Fix the system first. Then see who's still struggling.
